Right lung biopsy from a 61 year old male with a large hilar mass. Question large cell lymphoma (from accompanying requisition): While lymphomatoid granulomatosis is considered a subtype (or within the spectrum of) diffuse large B-cell lymphoma, it has it’s own designation within the WHO classification of B cell malignancies. It is an angiocentric/angiodestructive lymphoproliferative disease composed of large EBV positive B cells and reactive T cells. Although prominent necrosis and angiodestructive lesions are not seen in this case, the biopsy specimen was small and necrotic areas may not be represented. There are 3 grades based on the number of large EBV positive cells relative to the reactive background.
